Beginning a New Home Education Program: Required Documents Listed Below:
*Download and complete the Milton Area School District Notarized Affidavit*
Provide educational objectives by subject matter
Provide evidence of immunizations
Provide evidence of yearly Health/Vision and Medical Services – Health and Medical Services – List
Supervisor – Provide evidence that the supervisor possesses a high school diploma or GED.
The supervisor is the parent/guardian or person having level custody of a child. S/he is the person responsible for providing instruction.

*District Approved Home Education Notarized Affidavit:
Prior to the commencement of the home education program, and annually thereafter on August 1, the parent/guardian or other person having legal custody of the child or children shall file the district approved notarized affidavit with the Superintendent setting forth the information required by law. “Supervisor” shall mean the parent or guardian or such person having legal custody of the child or children who shall be responsible for the provision of instruction, provided that such person has a high school diploma or its equivalent.
*Required Medical Forms to Submit:
Option 1: Complete the first three forms (vision, medical, dental) with doctor’s signatures and submit to the MASD District Office.
Option 2: Complete the medical exemption form and submit to the MASD District Office.
*You must choose either Option 1 or Option 2.
**Submit all documents to the Milton Area School District office located at 700 Mahoning St., Milton, PA 17847.

The District Office shall receive all required documents by their due dates.
